  • Monitor and manage the care of patients with long-term cardiovascular conditions such as hypertension, chronic heart failure, or Arrhythmias
  • Identify, assess, investigate, diagnose, and manage digestive symptoms
  • Identify and manage Acute Kidney Injury, including taking early action, such as stopping medications to reduce Acute Kidney Injury risk. Ensure case stability before discharge or referral to secondary care
  • Manage and diagnose common endocrine diseases such as diabetes mellitus and thyroid disorders.
